| This
system model portrays a few basic design elements of a nuclear power plant.
From a systems engineering perspective, the design function of the Pressurized Water Reactor (PWR) is to create steam, which will drive a turbine and generate electricity. A failure mode and effects analysis reveals that the design element most likely to fail is the water pump. A root cause analysis reveals that the pump can fail due to mechanical wear of the impeller and the drive shaft. A worst-case scenario pump failure could cause a Lose of Coolant Accident (LOCA) and require reactor shutdown. |
